Welcome to 2005 Northeast Dive Challenge.  If you wish to participate you must fill out the application shown below.  If you click on the form below to print a PDF version.

The enrollment is a two step process as follows:

Step one - All prerequisite certifications MUST BE MET prior to starting Team Success_tm Program.  See below!

Step Two - MUST SUCCESSFULLY COMPLETE the Team Success_tm Program.

The first 60 divers completing the Team Success_tm Program will be eligible to participate in the 2005 Northeast Dive Challenge forming 10 teams of 6 divers each.  Additional divers completing the Team Success_tm Program will form backup teams for the Dive Challange_tm Program and will be eligible for entry into future dive challenges.

Prerequisite certification(s):

PADI Advanced Open Water Diver or another training agency’s equivalent certification,

PADI Underwater Navigator specialty or another training agency’s equivalent certification

PADI Search and Recovery Diver or another training agency’s equivalent certification. 

(Diver’s participating in Team Success MUST BE certified in these three courses.)  Peak Performance Buoyancy or another training agency’s equivalent certification is highly recommended.  The Teaching status Team Success_tm Instructor is to ensure that the individual has the appropriate certification.
